Use of DD Form 2807-1 (Mar 2007), DD Form 2807-2 (Mar 2007), and DD Form 2808 (Oct 2005) is mandated for medical examinations (full physicals) instead of the SF88 and SF93. The ONLY exception where the SF88 (Oct 94) and SF93 (Jun 96) may still be used is for aviation medicine.
All previous form stock shall be destroyed to avoid confusion and errors. Local forms and checklists should be updated to reflect the changes.
Please be advised that as of today (June 2008), several of the individual forms provided by MRRS are incorrect and require revision. Thus several MRRS forms should not be used for medical examinations until the system is updated and the problems are corrected. Checklists for some schools or special duty assignments may also continue to call for the SF88 and SF93 in error.
